Our palliative care team is focused on improving quality of life through early interventions, symptom management, and support tailored to each patient’s needs. By addressing side effects of diseases and treatments early, we aim to reduce hospitalizations and provide greater comfort and stability at home.
Palliative care is especially beneficial for patients with serious or life-threatening conditions or those who have experienced multiple hospital stays in a short period of time.

Our mission is to walk alongside you in this journey, providing compassionate, personalized support that prioritizes comfort, dignity, and peace of mind. With a focus on improving quality of life, we partner with patients and loved ones to ensure care is tailored to their goals, easing this transition through attentive medical guidance, emotional support, and proactive interventions.
- Regular home visits every 1–3 months, with additional visits as needed
- Prompt response for worsening or new symptoms
- Advanced care planning and goal-setting with patients and families
- Symptom management tailored to each diagnosis
- Ordering diagnostic tests and additional services (PT, OT, HH, etc.) as needed
- Patient, family, and caregiver education
- Early interventions to prevent hospitalizations
- Deprescribing when appropriate, aligned with the patient’s goals

Palliative Care Referral Criteria
- Diagnosis of life limiting illness for symptom control, patient/family support
- Estimated prognosis of < 2 years
- Weight loss
- Progressive metastatic cancer
- 2+ hospitalizations for illness within three months
- Difficult-to-control physical symptoms
- Patient, family or physician uncertainty regarding prognosis
- Uncertainty regarding appropriateness of treatment options
- Patient or family requests for futile care
- DNR order conflicts
Conflicts or uncertainty regarding the use of non-oral feeding/hydration in cognitively impaired, seriously ill or dying patients - Limited social support in setting of a serious illness (no family or friends, overwhelmed family or caregivers)
- Patient, family or physician request for information regarding hospice appropriateness
Neurological Criteria
- Feeding tube is being considered for any neurological condition
- ALS or other neuromuscular disease considering mechanical ventilation
- Any recurrent brain neoplasm
- Parkinson’s disease with poor functional status or dementia
- Advanced dementia with FAST score of 6a or greater
